BOX  1923 ARCADIA.  FLORIDA  34265</B></div> <DIV ALIGN="LEFT"></DIV> <div> <B> </B></div> <bR> <bR> <div> <B>                               MATCH WAIVER</B></div> <bR> <div> WAIVER, RELEASE OF LIABILITY AND ASSUMPTION OF RISK, READ BEFORE SIGNING</div> <div> In consideration of the undersigned being allowed/permitted to use the Desoto Gun Club Inc.</div> <div> facilities, or to participate in any way in the Desoto Gun Club Inc. program(s), club sponsored</div> <div> events, other shooting events, and individual shooting on club premises, including scheduled</div> <div> shooting matches, the undersigned acknowledges, appreciates, and agrees that:</div> <bR> <div> <B>1. </B><FONT SIZE="3" COLOR="#FFCC66" FACE="Georgia" >The risk of injury from firearms use, and involvement in related activities or program(s) is</FONT></div> <div> significant, including the potential for permanent injury, paralysis, contracting a virus (such as</div> <div> COVID 19), or death, and while club rules are designed to eliminate and reduce potential harm,</div> <div> injury, or death, as is Club equipment, and personal discipline including shooting techniques,</div> <div> the risk of serious injury or death does exist.</div> <bR> <div> <B>2. </B><FONT SIZE="3" COLOR="#FFCC66" FACE="Georgia" >I the undersigned having read this entire Match Waiver and with a full understanding of it’s</FONT></div> <div> implications KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and</div> <div> unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE OTHERS, and assume full</div> <div> responsibility for my participation in club sponsored events and individual use of Club facilities.</div> <bR> <div> <B>3. </B><FONT SIZE="3" COLOR="#FFCC66" FACE="Georgia" >I the undersigned willingly agree to comply with Club rules, regulations and any customary</FONT></div> <div> terms and conditions for participation in any Club activities. If, however, I observe any unusual</div> <div> significant hazard or dangerous behavior by others during my presence or participation, I will</div> <div> remove myself from participation, and bring such to the attention of the nearest Club Officer,</div> <div> Range Safety Officer, or match director immediately.</div> <bR> <div> <B>4. </B><FONT SIZE="3" COLOR="#FFCC66" FACE="Georgia" >I the undersigned for myself, and on behalf of my family, heirs, personal representatives,</FONT></div> <div> and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Desoto Gun Club Inc. their</div> <div> officers, officials, agents, and/or employees, ALL other participants, club sponsors, event</div> <div> sponsors, with respect to any and all injury, disability, death, or loss or damage to person or</div> <div> property, WHETHER ARISING FROM THE NEGLIGENCE OF OTHERS OR OTHERWISE, to</div> <div> the fullest extent permitted by law.</div> <bR> <div> I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT.
